A review into one of England's worst performing hospitals has found serious failings in surgical care and a toxic workplace culture.
Inspectors said care was unacceptable in nearly half of cases reviewed at the Queen Elizabeth Hospital in Kings Lynn.
The trust has apologised, paused robotic surgery and says new leadership and safety measures are now in place.
