An investigation is underway after the Norfolk and Norwich University Hospital released a deceased patient to the wrong funeral directors.
The trust has apologised to the family, blaming human error and a failure to follow procedures.
The body was returned once the mistake was identified.
Extra checks and upgraded IT systems are being introduced to prevent further incidents.
